The Colorado Division of Insurance must be able to contact its licensed producers. All licensees must provide a contact address to the Division of Insurance upon licensure. Renewal notices are sent to the address listed on Division records.

Change of Address Notification:

  1. Individual and insurance agency producers must inform the commissioner in writing, in a form determined by the commissioner, of any change of address within thirty (30) days of the change.
  2. Failure of any licensee to inform the commissioner of any change to the licensee’s address of record or residence address will be grounds for a penalty.

As of August 1, 2010:

  1. The Division will impose a minimum fine of $100 for failure to notify the Division of a change of address within thirty (30) DAYS.
  2. An applicable surcharge (currently at 10%) will also be imposed.
  3. The Division may also impose other penalties including suspension and revocation of the producer license.
