The Kentucky Insurance Department requires all persons holding licenses to notify the department within 30 days of any change in any address (residence, business/home-office, mailing addresses and email address).  Persons failing to comply are subject to a $250 fine.

Penalties may be increased for multiple violations. Penalties may be imposed if any address on file with the Department is determined to be insufficient or invalid.

Changes must be made by logging into the e-Services account on the Department’s website at http://insurance.ky.gov/. Click on eServices in the upper right corner.

To set up an eServices account, click on First Time Here?, under the log in box. Once logged in, click on Record Correction Request (Form 8303 / Address / SSN /Name / Email / Phone) and make the necessary changes.

The Department will not accept changes by phone or e-mail, etc.  All corrections must be made through the eServices account.

NOTE:  It may take up to 48 hours for the changes to show up on the account.
